Admission Nurse

The RMC Admissions Nurse coordinates with the Emergency Department Team Leader or Charge Nurse inpatient admissions or observation stays. The Admissions Nurse will also work with the AURS team, Cath Lab, or Outpatient Surgery to identify any direct admission in process or pending admission. The Admissions Nurse will handle the entire admission process from time of referral to actual move in to ensure smooth transition during move-in process. The Admissions RN is responsible for communicating information to MDS/IDT team to determine RUG categories, level of care, and payment source. Responsible for screening, preassessment and completion of admission papers. The Admissions RN is responsible for acquisition of equipment, supplies, and notification of pharmacy meds. Maintains ongoing audit of current records and charts to ensure completenes and compliance with regulations. Completes initial physical admission assessment and documentation of plan of care. Evaluates and conducts resident/family education needs for diagnosis and discharge planning.



PART III: POSITION RESPONSIBILITIES (ESSENTIAL FUNCTIONS)Assist with patient placement.Assure that the patient is properly banded for falls and allergies.Complete the admission history and facility screens.Complete the medication reconciliationHave the patient and/family sign the falls agreement.Implement the appropriate falls precautions.Implement the core measure check list (if appropriate).Mentors others in all aspects of nursing practice.Obtain Physician orders (minimal transitional order set).Provide the patient with the admission packet.


Experience/Education Requirements:

+ Graduate of an approved school of nursing, BSN preferred. As of 1/1/18, must complete BSN within 4 years of hire or transfer.

License or Certification Requirements:

+ Current Illinois RN license

+ Current CPR certification or obtainment within 6 months of hire date with subsequent recertification annually.
